Two care homes in Wales are to deploy a remote patient monitoring platform to help improve the wellbeing of residents and reduce the risk of hospital admission.
NSW Health wants the virtual care strategy to cover care planning and coordination across different providers; clinical collaboration, innovation and research; patient self-management; and a digitally-capable workforce.
